Rosalie Hall is a Young Parent Agency and accredited Children’s Mental Health Centre dedicated to providing specialized care and treatment for young parents, their infants, and children. Our services include high quality learning and supportive child care program, infant mental health treatment, Live-In Treatment Residence, and a wide range of outreach programs for young families in need. Each year, we deliver evidence-based treatment to more than 1,000 individuals, with an additional 3,000 supported through outreach and drop-in services. Position Overview

The Live-In Treatment Case Manager is responsible for the effective planning, organization, and delivery of residential programming in collaboration with the residential team. This position provides direct supervision and care to residents and their infants, ensuring compliance with Rosalie Hall policies, procedures, and standards of practice.

Summary of Duties

  • Collaborate with the residential team to plan, organize, and implement programming that enhances service delivery and work processes.
  • Provide direct supervision and care of residents and their infants in accordance with Rosalie Hall policies and procedures.
  • Deliver counseling, guidance, and support to residents individually and in group settings.
  • Implement medical instructions from the clinic as directed and document in compliance with Rosalie Hall and Ministry requirements.
  • Maintain thorough and accurate documentation, including case notes, logs, and progress reports, to support service delivery and care planning.
  • Ensure resident and facility safety through nightly rounds and bed checks.
  • Communicate pertinent information to team members to support the development and achievement of individualized Plans of Care.
  • Assume coordinator responsibilities when assigned.
  • Educate residents about community resources and encourage their active use of supports.
  • Contribute to the learning environment by providing guidance and support for clients.
  • Identify, intervene and deescalate crisis situations and work with clients to develop crisis management skills.

General

  • Use community development and conflict resolution approaches to support clients to develop skills to live cooperatively with others.
  • Demonstrate respect for, and expertise in working with, clients experiencing mental health challenges, addictions, abuse, and social isolation, including those from racially and ethnically diverse backgrounds.
  • Maintain all aspects of confidentiality relating to clients, volunteers, staff, programs, community members and community partners
  • Maintain a calm, welcoming, empathetic and professional demeanor.
  • Be available to provide shift coverage in emergency situations, including overnights, in support of the 24-hour operations of the Live-In Treatment (LIT) facility.

Qualifications/ Skills

  • CYW diploma or equivalent
  • Comprehensive understanding and knowledge of the mental health, addictions, health sector, and community resources
  • Solid understanding of the intersectionality of systemic barriers and social determinants of health, including racism, discrimination, and oppression
  • Ability to work independently and under pressure as an effective team member
  • Excellent oral and written English communication fluency
  • French or other second language(s) is a strong asset
  • Training in motivational interviewing and evidence-informed interventions is an asset
  • Successful police services Vulnerable Sector Screening Check
